Some objects have special meanings when it comes to tattoos. For example, candles often represent light over dark and good over evil. That symbolism, combined with the realistic design of the woman and the skull, makes us believe that this person wanted to convey a message related to life winning over death.
Apart from admiring this piece, one can only imagine how painfully excruciating it would be to get a tattoo such as this one. People say fingers hurt more than other placements because there is less muscle and fat tissue. Nonetheless, the result is fantastic, so the pain was probably worth it.
